Application Process for Work Permit

  1. Complete the Application:

    • Fill out all required sections of the application form.
  2. Obtain Signatures:

    • Parent/Guardian Signature:
      • Have a parent or guardian sign in the designated space provided on the form.
    • Employer Signature:
      • Have your employer sign in the designated space provided on the form.
  3. Return the Completed Application:

    • Scan or take clear photos of the signed application.
    • Email the completed application to sortega@mitacademy.org at your earliest convenience.
    • Drop off physical copy to Mr. Ortega in Main Office
  4. Processing/Receive Work Permit:

    • Work permit will be send to you.
    • Present the official work permit to your employer.
